To our knowledge, arterial compliance of fetal circulation is mostly unknown. We tested whether arterial compliance differs between male and female fetuses. We measured the carotid-femoral pulse wave velocity (cfPWV) in 53 male and 64 female fetuses during the mid-trimester (gestational weeks from 24 to 28) in a low-risk Japanese population. Carotid-femoral PWV is widely used to assess the value of arterial compliance in the general population (10, 20, 22, 38).

cfPWV as a surrogate cardiovascular biomarker reflects arterial stiffness and intrinsic cardiovascular properties. Because cfPWV is characterized by the distance (D) between the pulse wave traveling along the common carotid artery and aorta and the time it takes for the pulse wave to arrive from the carotid to the femoral artery (T) (1), cfPWV is calculated as follows: cfPWV = (D/T) x 0.8397. In this study, we used a noninvasive and validated model from the device (SAFIRE Vx; Toshiba Medical Systems Corporation, Tokyo, Japan) to measure blood pressure of fetal arteries with the participants atop the bed, which produced a fetal blood pressure profile. CFWV was calculated from the blood pressure waveform with the participants’ femoral artery and carotid artery waveforms.

After delivery, the infant’s age, weight, length, and head circumference were recorded. During physical examination by a physician, body temperature, heart rate, and blood pressure were obtained. Fetuses with asphyxia, defined as a blood gas pH value of <7.0 or umbilical cord artery blood gas pH <7.1 (10, 14), were excluded from the analyses. Fetuses were placed in the supine position with their arms by their sides and the head directed toward the side of the Sonde® (Toshiba Medical Systems Corporation) transducer. A 4-mil (0.2 mm) wide belt was wrapped around the fetal abdomen, with the distal fetal end of the transducer attached to the central belt.
